The RP-5217-PDF is a web-based downloadable, barcoded, pdf document. (It replaces the RP-5217 four-part paper carbonless form.) Form preparers will enter the appropriate information into the form and should save the document to their PC until the transfer has been completed and the deed filed with the County Clerk.

File management

Creating a directory folder and using standard naming conventions to uniquely identify each RP-5217-PDF - Real Property Transfer Report is highly recommended (i.e., My Documents/RP-5217/SmithToJones). Changes to that document can then be made up to the point where the document is printed for signatures prior to the closing and / or deed recording. Please print the form as an 8 ½ x 14 inch legal size document. The signed original RP-5217-PDF must accompany all deeds and correction deeds upon filing with the Recording Officer. A filing fee is also required.

Limited data items (date of Sale, full sale price, spelling error, etc) may require a change AFTER the form is filled and signed.  Please draw a line through the item and print the updated information above the original computer entry.

To ensure access of the most current version of the RP-5217-PDF document, it is highly recommended that the user return to this web page each time a new document is created. However, if a user chooses to save a blank RP-5217-PDF to their hard drive or network and use that as their master / shell, they may do so with associated risks of not using the most current version of the form, which may result in form rejection when recording the Deed. Barcode

Please note that the barcode found at the bottom of the form, which captures all data entered by the user, is recalculated each time the information is changed / added / deleted. The form must be saved each time the document is re-opened and changed in order to capture the changes.  This feature allows data changes to be captured right up to the point where the document is printed for signatures prior to the closing and / or Deed recording. The bar-coded data provides the mechanism to share and transmit data between parties who, in the past, received data via the "four-part" paper copy of the RP-5217. Therefore, it is imperative that the transfer information be complete, accurate and entered from a PC into the document.

Assessment information

Although there are many built-in edits within the form, please use the link to RP-5217.pdf Instructions, FAQs and Municipal Contact Information for assistance to ensure accuracy and completeness of information being entered with particular attention being given to Items 16 through 20. The appropriate information must come from the latest tax bill at time of sale / transfer or the latest final city or town assessment roll. The Tax Map ID / Roll ID (Item 20) is a unique number that distinguishes each parcel in a municipality. It is very important to transcribe it exactly as it appears, including dots and dashes (i.e., 087.77-1-22.2). However, the six digit SWIS code (i.e., 555689) is not part of the tax map identifier and should NOT be included.

Also, the Tax Map Identifier/Roll Identifier entries (Item 20) are related to "the number of Assessment Roll parcels transferred on the deed" (Item 4).

In the example above, one parcel is being transferred. Therefore, the Tax Map ID related to that parcel would be entered on the first line in Item 20. If three parcels were being transferred, then the first three lines in Item 20 would contain the three identifiers for the associated parcels.
